Fat is all the rage right now. We’re losing it with weight-loss medications, getting rid of it via liposuction, and then scrambling to put it where we actually want it. It’s almost like it’s become less about the fat and more about strategic placement and balanced proportions.

Now there’s a new player in the fat game: AlloClae, an injectable body filler that uses donor fat to add volume to key areas, like the hips or buttocks. The concept is similar to traditional fat transfer, which uses your own fat to sculpt refined curves, but one can be injected in under an hour without surgery (AlloClae), while the other calls for liposuction first (fat transfer).

If you’re trying to figure out which approach makes the most sense for you, you’re in the right place. At Hakimi Plastic Surgery, we’ll walk you through exactly how AlloClae and traditional fat transfer compare so you can make the best choice for your body and lifestyle.

What Is Fat Transfer? What Is AlloClae?

Both procedures add natural volume, but they take different paths to get there.

Fat Transfer

Fat transfer (also called fat grafting or autologous fat transfer) involves taking fat from one area of the body where it’s not wanted and adding it to another. Dr. Hakimi uses liposuction to collect fat from areas like your abdomen, thighs or flanks, then carefully injects it where you want more volume. The breasts, buttocks, hips and face are some of the most commonly treated areas.

The appeal is simple: it’s 100% your own tissue. No synthetic materials, no implants, just your own fat being relocated to create better proportions. For patients who value natural results, this approach feels authentic, but it does involve liposuction and everything that entails, including general or local anesthesia, the time it takes to harvest fat, and some amount of downtime.

AlloClae

AlloClae is a newer option in aesthetic medicine, classified as an off-the-shelf structural adipose filler. What exactly does that mean, you ask? Let’s break it down.

  • Off-the-shelf: AlloClae uses radiated (purified) donor fat cells to add volume without a round of liposuction first. The fat cells are already in the syringe, ready to go, making it a convenient “off-the-shelf” option. No surgery, no incisions, and minimal downtime.
  • Structural: AlloClae is purified to remove any DNA but retains the honeycomb-shaped “fat matrix” that acts like scaffolding for extra support. The donor fat adds immediate volume, while the scaffolding integrates with the body and stimulates new tissue growth.
  • Adipose filler: AlloClae can be thought of as a fat (adipose) filler for the body, especially for the hips, breasts and buttocks. The treatment process is quick and convenient, requiring only local anesthesia and taking less than 30 minutes to complete.

AlloClae is FDA-compliant and manufactured under strict conditions. We say FDA-compliant because, as a human tissue-derived product, it doesn’t require FDA approval or clearance the same way drugs or medical devices do. The fat is radiated to remove all genetic components, so once it’s injected into your body, it becomes more yours than the donor’s.

Key Features & Benefits

Fat Transfer

Features:

  • Uses 100% of your own fat for natural results
  • Can achieve more dramatic volume increases if sufficient fat is available
  • Slims donor areas through liposuction
  • Permanent results once the transferred fat establishes a blood supply
  • Ideal for larger volume goals in the breasts or buttocks
  • Can be used in the body or face

What’s Involved:

  • Liposuction and general or local anesthesia (awake lipo makes this process easier)
  • Two surgical sites (donor and recipient areas)
  • Initial results visible as swelling subsides
  • Final results develop as transferred fat becomes established
  • Not all transferred fat survives; absorption rates range from 20% to 50%
  • Awake lipo with Dr. Hakimi involves minimal (two-day) downtime

AlloClae

Features:

  • No liposuction required; donor fat is ready to be used
  • Quick procedure (typically 30 to 60 minutes)
  • Provides subtle volume to fill in hip dips, round the buttocks or smooth breast contours
  • Perfect for patients without enough body fat to harvest
  • Lower resorption rates compared to traditional fat transfer
  • Only suitable for the body; AlloClae is “too big” for facial fat grafting

What’s Involved:

  • Performed in-office using only local anesthesia
  • Single injection site per treatment area
  • Immediate volume that continues to improve over the following months
  • May require multiple sessions for optimal results
  • Minimal downtime; most patients resume normal activities in 24-48 hours
  • Fat behaves like your native tissue for long-lasting results

Comparing Results: What Can You Expect?

The results from both procedures look natural, but the degree of enhancement differs.

Traditional fat transfer delivers more substantial volume, with a typical BBL adding 800-1000 cc of fat to each buttock. If you have adequate donor fat, you can achieve significant enhancement in a single session. The transferred fat that survives becomes a permanent part of your body, responding to weight changes just like your other fat cells. However, some resorption is normal, which means you might need a secondary procedure to achieve your ideal outcome.

AlloClae provides more subtle volume, typically adding about 50-100 cc of fat in key areas. You can expect to smooth away contour irregularities in the breasts or hips, or to see a soft rounding of the buttocks, but the goal is “gentle curves” rather than dramatic transformation. The donor fat has been processed to maximize retention, so more of what’s injected tends to remain. Many patients also appreciate the gradual improvement in skin texture and quality that develops over time as the AlloClae stimulates natural regenerative processes.

Which Procedure Is Right for Me?

Both procedures have earned their place in body contouring. The question isn’t which one is objectively better. It’s which one is better for your specific situation.

When to Choose Traditional Fat Transfer

Traditional fat transfer makes the most sense if you have these priorities:

You prefer using only your own tissue. For many patients, this is the deciding factor. Using your own fat means nothing foreign enters your body, which can provide peace of mind.

You want more substantial volume enhancement. If you’re looking for a noticeable increase in breast size or buttock projection, traditional fat transfer can deliver more dramatic results.

You have sufficient body fat to donate. This procedure requires adequate fat reserves. If you’re already at a lower BMI, you may not have enough fat for the volume you want to achieve.

You don’t mind a longer recovery period. If you can take some time off from strenuous activities, the recovery process shouldn’t deter you. Fortunately, Dr. Hakimi’s advanced awake liposuction techniques translate into just two days of downtime for most patients.

You’re combining procedures. If you were already considering liposuction to slim certain areas, fat transfer allows you to repurpose that fat rather than discarding it.

When to Choose AlloClae

AlloClae becomes the better option in these scenarios:

You don’t have enough body fat for traditional transfer. This is where AlloClae really shines. Lower BMI patients who would struggle to donate sufficient fat can still achieve beautiful results.

You want minimal downtime. If you can’t afford any downtime or you simply prefer to avoid a more invasive procedure, AlloClae’s quick recovery is appealing.

You prefer subtle curves. If you’re looking for subtle changes rather than dramatic transformation, AlloClae offers refined results that enhance your natural proportions.

You’re correcting minor contour irregularities. AlloClae works beautifully for smoothing hip dips, evening out breast contours or adding subtle fullness to specific areas.

You value convenience. The in-office procedure takes about an hour, and you walk out the same day ready to resume most normal activities.
